Needs assessment and service coordination

The primary point of contact for services for older adults is Needs Assessment and Service Coordination (NASC). NASC services cover help to stay at home, support services for carers and entry into residential care. They can also link with other District Health Board (DHB) services and community based agencies.

  • Meals on Wheels: Access to Meals on Wheels service requires referral by a medical professional. For existing clients wishing to cancel or place a temporary hold on meal delivery, phone 04 918 6014 and leave a message.
  • District nursing: The full service operates 8am-4.30pm, Monday to Friday, with reduced staffing in the evenings and weekends. An on-call service operates between 10pm-8am. Wellington, 806 2556. Kapiti, 903 0224. Kenepuru 918 2011.
  • Shuttle services: Shuttle service() operates between Wellington and Kenepuru hospitals.
